Recurring cleaning goes wrong in small ways: the oven nobody said was extra, the key that didn't work, the vase that was already chipped. A one-page agreement saves the account before any of that comes up.
The agreement

Recurring Cleaning Agreement between [Your Company] ("we") and [Customer Name] ("you") for the home at [Service Address]: [number] bedrooms, [number] bathrooms, about [square feet] sq ft.
1. Schedule. Cleaning [weekly / every other week / every 4 weeks] on [day], arriving between [time] and [time]. The first clean is a deep clean, priced separately at $[amount].
2. Each clean includes. Kitchen: counters, sink, stovetop, outside of appliances and cabinets, microwave inside and out, floors. Bathrooms: tub, shower, toilet, sink, mirrors, floors. All rooms: dust reachable surfaces, wipe switches and handles, vacuum, mop hard floors, empty trash, make beds [with linens you leave out].
3. Not included unless added. Inside the oven or refrigerator, inside cabinets, interior windows, walls and baseboards (deep clean only), laundry, dishes, organizing, pet waste, and anything that needs a ladder. Add-ons and prices: [list].
4. Supplies. We bring our own supplies and equipment [except: (items you want us to use)]. Tell us about any surfaces that need special products (marble, natural stone, unsealed wood).
5. Price and payment. $[amount] per clean, billed [after each clean / monthly], due within [number] days. Our price assumes the home is kept at about the same condition. If it changes a lot, we will talk to you before changing the price.
6. Entry. You will let us in by [key / lockbox / door code / being home]. Keys and codes are kept [labeled by code, not by address] and returned when this agreement ends. Alarm code: [given separately, not written here].
7. Lockouts and late cancellations. If we can't get in on the scheduled day, or you cancel with less than [number] hours' notice, a fee of $[amount] applies.
8. Skips and holidays. Tell us [number] days ahead to skip a clean. If a clean falls on [holidays], we will offer the nearest open day.
9. Breakage and damage. Tell us within [number] hours if something is damaged during a clean and we will repair or replace it, up to $[amount] per item. Please put away items that are fragile, valuable or irreplaceable. We are not responsible for items already broken or loose, or for wear from normal cleaning.
10. Pets and safety. Pets should be [crated / put in a closed room] during the clean. We may leave and charge the lockout fee if conditions are unsafe.
11. Cancellation. Either of us may end this agreement with [number] days' notice. Cleans done up to that date are billed.
