Pest control agreement with the re-service promise in writing.
Quarterly pest plans live or die on two promises: what bugs are covered, and whether you come back free when they show up between visits. Write both down, along with the pests that are never part of the plan.
The agreement

Pest Control Service Agreement between [Your Company] ("we"), license no. [number], and [Customer Name] ("you") for the home at [Service Address].
1. Initial service. An initial interior and exterior treatment on [date] for $[amount], to knock down current activity.
2. Regular service. Exterior perimeter treatment every [3 months / (other)] after the initial service, with interior treatment when needed and when you ask for it. We will contact you before each visit. If you're not home, we will treat the exterior and leave a note.
3. Covered pests. Ants (except carpenter ants [unless listed]), spiders, cockroaches, silverfish, earwigs, crickets, centipedes, millipedes, [mice in living areas, with stations and traps billed as listed below], and [other].
4. Not covered. Termites and other wood-destroying insects, bed bugs, fleas and ticks [unless added], stinging insect nests over [height], wildlife (squirrels, raccoons, bats, birds), and structural exclusion work. These can be quoted separately.
5. Re-service. If covered pests come back between regular visits, call us and we will come back and re-treat at no charge.
6. Price and payment. $[amount] per quarterly visit [or $(amount) per month]. Rodent stations $[amount] each, traps $[amount] each, exclusion priced per spot after inspection. Billed [after each visit / monthly], due within [number] days.
7. Products and notice. We use products registered with the EPA and applied according to their labels and [State] law. We will tell you what was applied at each visit, and a copy of any product label is available on request. Let us know before service if anyone in the home is pregnant, has a chemical sensitivity, or keeps fish, reptiles or bees.
8. Your part. Give us access to treated areas, keep children and pets off treated surfaces until dry, store food and dishes away before an interior treatment, and fix moisture problems and food sources we point out. Treatment results depend on these.
9. Term and cancellation. This agreement runs for [12 months] from [start date] and continues after that until either of us cancels with [number] days' written notice. Visits done up to that date are billed.
10. Liability. No treatment can guarantee that a home will stay completely pest-free. Our liability is limited to re-treatment or a refund of the last visit, except where the law in [State] does not allow that limit.

Running the plan in Redline

Re-service calls are one-offs: "find a time for a Delgado re-service this week" offers open slots, favoring days you're already in her town.
